I was pleased to have the chance to respond to the excellent letter from Gurlek et al.

My article described a new technique. It is fascinating that Gurlek and colleagues have been using a similar technique to correct the crooked middle third of the nose.

They modify the shape of the graft at each end to provide a better fit. Changing the shape at the bony end may help the graft fit better under the nasal bones. Changing it at the caudal end reduces risk of graft exposure in the nostril. They make valid points regarding these modifications. The 0.85-mm grafts are so thin that I have never found excessive widening with them.
