We read with interest the recent article by Mendelsohn.1 After reviewing our own cases, we would like to share our experiences and our modification of the graft shape (Figure) with other surgeons who might like to use this procedure.

First, we would like to congratulate the author on his idea and applications. For 16 months, we have been using high-density porous polyethylene (MEDPOR; Porex Surgical Inc, Newnan, Ga) as both spreader grafts and extended spreader grafts to straighten the noses of more than 40 patients.
