DR JASIN1 has written a good article concerning the management of the aging neck. It reiterates the principles and clinical applications elucidated in an earlier study on isolated platysmaplasty.2 The technique has evolved over the years and remains an important ancillary procedure for a successful aesthetic result in most face-lifts. As Jasin notes in his clinical review, an isolated platysmaplasty with appropriate fat removal is indicated for selected patients who have minimal or moderate platysmal banding. Because it is relatively minimally invasive and involves only limited dissection with few complications, it is a useful procedure.
The most important criteria for a successful result is a balanced and harmonious aesthetic appearance. The isolated platysmaplasty is not an alternative for face-lifting if the cervical skin is significantly redundant or if jowling is excessive.
