SOME TENETS of septorhinoplasty are relatively universally accepted
and include the concept that structural grafting is best done with autogenous
materials.1-4
The graft-depleted revision septorhinoplasty patient and other septorhinoplasty
patients with major structural grafting requirements provide unique challenges.
Many autogenous materials are useful in these cases and include septal cartilage
and bone, conchal cartilage, costal cartilage, rib bone and costal cartilage
combination graft, calvarial bone, and iliac crest bone. The article by Cakmak
and Ergin5 describes their experience with
20 cases in which costal cartilage was used as the primary structural and
nonstructural graft material.
As Cakmak and Ergin5 and others1, 3 have pointed out, costal cartilage
is available in abundant quantities and can be harvested with minimal morbidity.
As these authors describe, and as observed in my own experience, costal cartilage
is an ideal material for septal replacement grafts, spreader grafts, and columellar
